Housing Services:

Housing Stabilization REQUIREMENTS:

  • Eligible for Minnesota Medical Assistance

  • Have a housing need or are homeless

  • Are 18 years old or older

  • Identified disabling condition

  • Completion of a Housing Consultation

Housing TRANSITION/Stabilization Include:

  • Assist in finding sustainable housing prior to discharge from a facility

  • Assist in finding sustainable housing if homeless

  • Assist in maintaining housing

  • Skill building to improve communication between landlord and tenant to maintain housing

  • Work with the client in a team based approach to meet their unique housing needs

  • Assist in the completing of programmatic requirements to attain or maintaining housing  

Housing Supports (Clay County only):

  • Disabling condition identified

  • VI SPDAT completed

  • Long Term Homeless eligibility form completed (verifying 1 full year of homelessness, or 4 times in the past 3 years)

  • Eligible to receive MN benefits
