Mental health

We have recently expanded into providing a variety of outpatient Mental Health services. Our staff has expertise in trauma and individuals with dual diagnosis. Meet our wonderful providers here.

Telehealth

Telehealth services provide accessible and convenient healthcare solutions by connecting individuals with medical and mental health professionals through secure virtual platforms. These services offer Greater Minnesota residents the opportunity to receive quality care from the comfort of their homes, improving access to essential support, especially in rural or underserved areas.

 

Adult Day Services

We provide a center based semi-structured day service integrating; Community Involvement, Exercise, Pre-Vocational Services, and Therapeutic and Cognitively Stimulating Group.

Eligibility:

  • 55+ or Received Day Services prior to 1-1-21.

  • On a waiver or have a private pay agreement

  • Completed Application

Housing Services

We provide a variety of housing services in Clay, Becker, and Ottertail Counties.  This includes being a Housing Support Housing Case Manager Provider in Clay County, Providing Housing Consultations, Housing Stabilization, and Transition Services.

245D Home and Community Based Services

A 245D License focuses around a variety of Home and Community-Based Services (HCBS) provided to people with disabilities and those over age 65.  Services are individualized and focused around both client’s need and client’s choice of services
